This is a case study of violence and sexual abuse towards women with disabilities in Malawi. It is based on in-depth interviews and focus group discussions. This work examines the childhood and adult experiences of these women, and asserts that discrimination, stigma and vulnerability is worse for adult women.It claims that improvements to access and education will lead to empowerment, thus improving the overall quality of life that women with disabilities can enjoy. This article explains why, in the face of increased funding for malaria programmes, the disease incidence shows little signs of abating. In sub-Saharan Africa nearly a million people die as a result of malaria. The article draws attention to the cultural, social and economic contexts in which communities deal with the consequences of malaria. Health systems, services and infrastructures are generally inadequate and fail to deliver proper care. Malaria can also be perceived as a mild illness and used to hide more stigmatising health problems. The paper advocates focusing on the 'normality' of malaria, and exploring the social and economic contexts that shape household and community responses to malaria

Policy lessons of the East Asian demographic transition

The usual lessons drawn from East Asia's striking experience of health and fertility transition concern the efficacy of well-designed government programmes catering to an existing or ideationally stimulated demand. An alternative interpretation sees the demographic change and the uptake of services as a byproduct of social and economic development together with, in some cases, strong government pressures. This paper probes more deeply into this experience, seeking to identify common features of development design and administration that underlay it and to derive lessons for policies elsewhere. The broad sequence entailed, initially, establishment of an effective, typically authoritarian, system of local administration, providing (sometimes incidentally) a framework for promotion and service delivery in health, education, and family planning. Subsequent economic liberalization offered new opportunities for upward mobility and greater risks of backsliding but along with erosion of social capital and the breakdown or privatization of service programmes. The study is mainly focused on seven countries: Taiwan and South Korea ("tiger" economies), Thailand, Malaysia, and Indonesia ("second wave" countries), and China and Vietnam ("market-Leninist" economies). The period is roughly from the 1960s to the 1990s

Employment leads to independent living and self-advocacy : a comparative study of employed and unemployed persons with cognitive disabilities

[Authors' abstract] : The main purpose of this study was to measure the effect of employment on independent living and self-advocacy of persons with cognitive disabilities. The other purposes of the paper were to measure the effect of severity of disability and type of employment, on self-advocacy skills and independent living skills. A ten item-five point rating scale was developed to measure the independence level and self advocacy of persons with cognitive disabilities. A qualitative and quantitative study of fifty unemployed and fifty employed persons with cognitive disabilities was carried out. The results were statistically analysed and a significant difference was found between the groups, with the unemployed persons with cognitive disabilities scoring significantly lower in independent living and self-advocacy skills. Those in open employment showed more independent living and self-advocacy skills than those in group employment. The practical implications of the findings of this study are discussed
