Towards more inclusive practices: A Disability, Gender and Age Intersectional Resource

This Disability, Gender and Age Resource aims to support staff to better understand intersectionality. An intersectional approach reminds us of the need to look deeper at the way multiple individual characteristics and societal factors intersect to compound discrimination in any given context. This resouce is split into w main sections:

 

In Section A, we introduce the concept of intersectionality, its use as a lens to understand vulnerability and the relevance of ‘context’. Section A also introduces a few critical concepts: the fact that disability, gender and age are all social constructs, the centrality of power and the need to transform unequal power relations.

 

In Section B, we provide some guidance on inclusion and bias; the need to consider the wider environment; how to work with social norms; how to understand power differently; and empowerment and participation processes.

Disability inclusion in shelter and settlements programming

The Global Shelter Cluster (GSC) Working Group on Inclusion of Persons with Disabilities in Shelter Programming was set up to build on existing guidelines and work addressing vulnerability factors to integrate a wider and more systematic disability inclusive approach to shelter and settlements programming, as well as support more general inclusion mainstreaming as prioritized by the GSC Strategy 2018 – 2022. To begin this process, the Working Group undertook a Baseline Mapping of how disability mainstreaming and inclusion is currently addressed within the sector. The Baseline Mapping included review of published literature and project documents, key informant interviews and an online questionnaire.

‘Enabling Access’: A Pilot Study on Access and Use of Assistive Products in the Northern Province, Sri Lanka

Purpose: The need for suitable assistive technology is growing all over the world, not only for people with disabilities but also for the ageing population with functional decline and non-communicable diseases. Access to assistive technology promotes access to education, employment and active societal participation. The aim of this study was to assess the self-reported need by persons with disabilities and by people who were 65 years and older without disabilities in the Northern Province of Sri Lanka, for assistive products; and to identify barriers to accessing these assistive products.

 

Method: This mixed-methods pilot study included 76 participants who were either persons with disabilities or their caregiver or persons 65 years and older, from the Northern Province of Sri Lanka, affected by the now-ended 30-year civil war.  To ascertain trends in the local need for assistive products, a translated version of the World Health Organisation’s Priority Assistive Products List of 50 items was used. In addition, semi-structured interviews with key participants were conducted, to gain some insights into the barriers to accessing assistive products. 

 

Results: The most widely used assistive products among persons with disabilities were connected to war-related injuries. In contrast, those used by the older age group of persons without disabilities were connected to non-communicable diseases and age-related frailty. The assistive products requested by both groups were aids to promote independence in daily activities and to support access to education and employment. The emergent themes included affordability, employment, independence in activities of daily living, stigma and psychological impact, and a lack of awareness and guidance in the use of assistive devices.

 

Conclusion: The findings highlight the need for policies and practices to be informed by local socio-cultural, historical and geographical realities.

Rapid review of disability and older age inclusion in humanitarian WASH interventions

A knowledge gap in good practices and innovation for how people with disabilities and older people are included in water, sanitation and hygiene (WASH) interventions in humanitarian contexts prompted this review. To promote inclusive humanitarian action, the Age and Disability Capacity Programme (ADCAP) consortium developed the Humanitarian Inclusion Standards (HIS). The HIS consists of nine key inclusion standards and sets sector specific standards, including for the WASH sector. The WASH inclusion standards are structured around three key dimensions of inclusion: 1) Collection of Information, 2) Addressing Barriers and 3) Participation and Resilience. This report provides key actions, trends and gaps for each of these dimensions. The report is based on a literature review and a small number of key informant iterviews.

Social participation and inclusion of ex-combatants with disabilities in Colombia

The purpose of this paper is to explore ex-combatants’ understandings of disability and the pathways for social reintegration available to them in Colombia. The qualitative data for the study include seven in-depth interviews with ex-combatants and 29 with key informants, including disabled people’s organisations, government agencies, international organisations and academic groups. Findings suggest that transition to civilian life for ex-combatants is made more difficult by inadequate procedures, lack of support and complex administrative data vacuums. Social determinants, historical prejudice against persons with disabilities, high levels of unemployment and political polarisation in a post conflict context combine to trigger poverty traps. The findings indicate pitfalls in the early implementation of the Colombian peace process, which did not consider structural issues that affected transition to civilian life for ex-combatants with disabilities. Furthermore, key enablers for social inclusion such as peer-to-peer support have been identified by respondents. This paper concludes that more needs to be done to enhance the voices of ex-combatants with disabilities and to understand the profound meaning of acquiring impairments through participation in conflict, as well as how post-conflict responses could enable these individuals to gain the skills they need to successfully reintegrate into their communities.

Childhood disability in Turkana, Kenya: Understanding how carers cope in a complex humanitarian setting

Background: Although the consequences of disability are magnified in humanitarian contexts, research into the difficulties of caring for children with a disability in such settings has received limited attention.


Methods: Based on in-depth interviews with 31 families, key informants and focus group discussions in Turkana, Kenya, this article explores the lives of families caring for children with a range of impairments (hearing, vision, physical and intellectual) in a complex humanitarian context characterised by drought, flooding, armed conflict, poverty and historical marginalisation.


Results: The challenging environmental and social conditions of Turkana magnified not only the impact of impairment on children, but also the burden of caregiving. The remoteness of Turkana, along with the paucity and fragmentation of health, rehabilitation and social services, posed major challenges and created opportunity costs for families. Disability-related stigma isolated mothers of children with disabilities, especially, increasing their burden of care and further limiting their access to services and humanitarian programmes. In a context where social systems are already stressed, the combination of these factors compounded the vulnerabilities faced by children with disabilities and their families.


Conclusion: The needs of children with disabilities and their carers in Turkana are not being met by either community social support systems or humanitarian aid programmes. There is an urgent need to mainstream disability into Turkana services and programmes.

Disability-inclusive healthcare in humanitarian camps: Pushing the boundaries of disability studies and global health

A significant proportion of forced migrants live in humanitarian camps located in remote regions of the global South. Disabled persons have been historically neglected within camp programs across all service sectors, especially healthcare. This paper describes an exploratory research study on healthcare access for forced migrants with disabilities in the context of humanitarian camps. Based on the methodological framework of rapid ethnography, the research involved guided tours, community mapping exercises, ethnographic observations, and semi-structured interviews in a refugee camp in southern Africa. Additional key informants from other sites were interviewed remotely using a convenience sampling strategy. Several important insights emerged from the research including: misperceptions about the health-related needs of disabled persons, their specialized health needs falling outside the ‘social minimum’ of humanitarian healthcare, and concerns about distributional ethics in relation to disability-inclusive healthcare. The research also highlighted barriers and strategies for addressing disability-specific health needs given significant resource constraints in humanitarian camps. These findings are discussed in light of practical and theoretical challenges in the fields of disability studies and global health.

Facilitating disability inclusion in poverty reduction processes: Group consensus perspectives from disability stakeholders in Uganda, Malawi, Ethiopia, and Sierra Leone

This paper addresses the challenge of how to get disability on the development agenda in four African countries. We explored perceptions of what initiatives would most help in achieving disability inclusion in Poverty Reduction Strategy Papers (PRSPs), and identified factors that can either promote or hinder these initiatives. Stakeholders from Disabled People’s Organisations (DPOs), other civil society organisations (CSOs), development agencies, researchers and government ministries, participated in the Nominal Group Technique and Force Field Analysis procedures across Malawi, Ethiopia, Uganda and Sierra Leone. While each country had specific contextual factors, common ideas for promoting greater disability inclusion in PRSPs focused on policy action, the need for a stronger evidence-base, mechanisms for directly influencing the PRSP process, as well as strengthening central government and DPOs’ capacity in this regard. Common facilitators for these actions were seen as the existence of a national disability umbrella body, disability-specific legislation, named Ministries for Disability, ratification of the UN Convention on the Rights of Persons with Disabilities (UNCRPD), and the fact that disability was already mentioned (albeit inadequately) in some PRSPs. Common inhibitors included negative attitudes towards disability, poor capacity in DPOs and government ministries, poor policy implementation, little ‘domestication’ of the UNCRPD, little political will or consultation with people with disabilities, as well as aggregating disability with other vulnerable groups, a lack of research in the area and poor coordination between DPOs.
