This report describes the difficulties in communicating on HIV/AIDS in schools. It includes experiences from India and Kenya, and reports the findings of a survey carried out by ActionAid researchers in both countries in 2002. The research examines parental demand for HIV/AIDS education. It then explroes teh role that schools have in meeting this demand and other sources that young people might use to learn about HIV and AIDS. The final section places HIV/AIDS communication in the wider context of a crisis in education in resource-poor settings, and highlights some of the barriers or silences in communication around HIV/AIDS. Among its conclusions is the suggestion that HIV/AIDS education be placed in the context of the community

This work attempts to show how traditional Taita dance and music can creatively be utilised to sensitise and mobilise for social change in a rural community in Kenya. A horizontal model of communication aimed at steering clear of the conventional top-down models is proposed so that by empowering the rural people, it is anticipated that they will be able to transmit messages concerning reproductive health and HIV & AIDS prevention in a manner that makes the messages intelligible to them, and within themselves, for long periods of time such that it leads to individual and community behaviour change

This resource pack is a helpful tool aimed at services, organisations and health professionals involved in the voluntary counselling and testing (VCT) centres in Kenya. VCT services offer HIV testing to asymptomatic individuals, and pre- and post-testing counselling. Compliance with quality requirements in the provision of the service is crucial to the achievement of the objectives. The National Guidelines for VCT set minimum standards for the registration and accreditation of sites and the Liverpool School of Tropical Medicine has devised a comprehensive quality assurance system to accompany the upscaling of the VCT programme, with an emphasis on individual and community involvement. This resource outlines the lessons learned in the implementation of quality assurance in VCT, and includes useful training materials and tools such as self-assessment and service supervision survey forms for the monitoring and evaluation of service delivery quality

Hydrocephalus and spina bifida are life threatening conditions that often result in severe dsabilities. Risks are much reduced by immediate surgery and careful managment, but neither has been available for most of the sub-Saharan African population. This paper traces the growth of solutions and some socio-cultural resources that historically have supported family and community care for children with severe disabilities, mainly in Tanzania, and nearby countries. Some community-based rehabilitation (CBR) work with children with spina bifida and hydrocephalus is described, and challenges to the CBR approach are noted from the increased survival of people with disabilities requiring complex care. More appropriate information, recognition of indigenous knowledge, enlistment of community resources and financial assistance are needed to enhance the lives of East Africans with hydrocephalus, spina bifida and other severely disabling conditions

This report presents three initiatives, in Kenya, Tanzania and Uganda, that have used an integrated approach to HIV prevention and treatment, combining traditional and biomedical health systems. In resource poor countries, traditional healers become part of the solution, helping provide a comprehensive response to the challenges of the disease, particularly when provided with adequate training. The report shows that implementation of programmes of this kind at larger scale could have the potential of bringing appropriate AIDS information and effective treatment to isolated communities at little cost

The study compares children who were taught by preschool teachers trained in the two-year course run by District Centres for Early Childhood Education (DICECE) with those who had untrained teachers. The study, carried out in Embu District (Kenya), found significant differences between the two groups of children particularly in terms of performance in primary schools, with children cared for by DICECE-trained teachers faring better, and in relation to absenteeism, repetition and dropout rates

The first phase of the study, completed in May 2000, indicates that youth would like access to HIV testing and counselling services if the services are confidential and inexpensive and if the results are reported honestly

Using evidence to change antimalarial drug policy in Kenya

This review analyses the range and quality of the evidence base that was used to change the drug policy in Kenya from chloroquine to SP and examines the process of consensus building and decision making. The review illustrates the difficulties in translating sensitivity data with gross geographical, temporal and methodological variations into national treatment policy. The process was complicated by limited options, unknown adverse effects of replacement therapies, cost, as well as limited guidance on factors pertinent to changing the drug policy for malaria. Although more than 50% of the studies showed parasitological failures by 1995, there was a general lack of consensus on the principles for assessing drug failures, the inclusion criteria for the study subjects and the relative benefits of parasitological and clinical assessments. A change in international recommendations for assessment of drug efficacy in 1996 from parasitological to clinical response further perplexed the decisions. There is an urgent need for international standards and evidence-based guidelines to provide a framework to assist the process by which decision-makers in malaria-endemic countries can make rational choices for antimalarial drug policy change
