Bridging the gaps between research, policy and practice in low- and middle-income countries : a survey of health care providers

This article discusses the results of a survey to examine the gaps that continue to exist between research based evidence and clinical practice. Health care providers in 10 low- and middle-income countries were surveyed about their use of research-based evidence and examined factors that may facilitate or impede such use. The conclusion is that locally conducted or published research plays an important role in changing the professional practice of health care providers surveyed in low- and middle-income countries and increased investments in local research, or at least in locally adapted publications of research-based evidence from other settings, are therefore needed. Although access to the Internet was viewed as a significant factor in whether research-based evidence led to concrete changes in practice, few respondents reported having easy access to the Internet. This article assesses "the contribution of physical, mental, and cognitive chronic diseases to disability, and the extent to which sociodemographic and health characteristics account for geographical variation in disability." Using cross sectional surveys of people aged over 65 in seven low and middle income countries, the findings highlight specific associations of chronic diseases to disability and overall that dementia, not blindness, is overwhelmingly the most important independent contributor to disability for elderly people in low and middle income countries. "The aim of the present review is to summarise evidence regarding the efficacy of CBR in relation to one particular ‘high risk’ group of disabled children; children with intellectual disabilities (ID)...Only 10 studies were identified for inclusion in the review of research on the effectiveness of CBR for children and adolescents with ID and these are summarised in Appendix Two. An examination of reviews on the effectiveness of CBR for all people with disabilities points to two main reasons for this low level of evidence. Firstly, CBR has not been the subject of a significant amount of rigorous evaluation. Secondly, children and adolescents with ID have not been the recipients of significant amounts of CBR. We will discuss the reviews on the effectiveness of CBR generally and indicate what they say about ID before outlining the extremely small amount of information available on the effectiveness of CBR for children and adolescents with ID"

These case studies come from an initiative that supports non-formal education projects in Africa, South Asia, the Caribbean and Latin America. The long-term goal of these projects is to develop sustainable livelihoods for low-income, low-literate populations by addressing vulnerability to HIV and AIDS and drug misuse, a lack of education and social exclusion. Central to all of these projects are the creative and innovative methods used to communicate in a meaningful way, engage people and encourage their participation. The projects all focus on capacity building, empowerment, and creating learning opportunities. A DVD has been produced to accompany this publication

This issue of Early Childhood Matters presents a number of perspectives around children's transition from home environments to primary schooling and includes practical examples of initiatives in a number of countries, including the US, India, Uganda, Guatemala, Poland, Brazil and Israel. Articles look at transition and school readiness as a challenge and a learning opportunity rather than as a problem. In different contexts the emphasis of related early childhood programmes varies. The 'Parques Infantis' in Brazil focuses on children's rights, while in Guatemala and Uganda programmes reflect a need to promote local culture and support children learning the mainstream language. The Mississippi Delta Children's Partnership encourages interaction between pre-schools, primary schools and parents as a means to facilitate transition and supports school readiness through after-school programmes
